Applications are due November 18, 2009
Awards will be presented at the NACDEP Conference in Bloomington, MN, April 12-14, 2010
1. Complete this form for each award application or nomination.
2. Remember to select the appropriate category.
3. All requirements must be met to be eligible for the award.
4. NACDEP members must be in good standing to be eligible for the current year and prior year.
5. Submit all required and supplemental materials electronically.
- Submit a complete list of names for all members of the team awards.
- Up to two pages of narrative addressing the award criteria.
